The CoC housing solutions team consists of staff from multiple agencies working directly with chronically homeless individuals, families, and veterans experiencing homelessness. Partners include Housing Counselors and Case Managers, Centralized Housing Intake coordinators, outreach workers, shelter providers, Supportive Services for Veteran Families, Veterans Affairs for Homeless Veterans, program staff, veteran organizations and employment providers in the CoC.

The team maintains a By-Name List of all individuals experiencing homelessness and engages in case conferencing to collaboratively and proactively tackle homelessness in the region.
